Supervisors Mid Semester Internship Evaluation
As the intern's supervisor, you should complete the following evaluation at mid-semester. This evaluation is to gauge difficulties that you may be having with your interns. In addition to this evaluation, you should also submit a final evaluation at the end of the semester. This report is strictly confidential, so you should be candid about any difficulties you are having with the student intern. Please return to Dr. Emile Sahliyeh at sahliyeh@unt.edu after the student has completed fifty (50) percent of his/her required hours.
