Supervisors Final Internship Evaluation

(To be completed by intern's supervisor)

The University of North Texas is committed to excellence in education and research at all levels of our academic program. As part of this ongoing challenge to prepare tomorrow's leaders, we appreciate your participation in our student intern program. Students must be evaluated to determine whether they fulfilled your expectations and to assess what areas need improvement. Please take a few minutes to complete the following.

Attach sheets that address questions 5-15. It is due no later than the Friday of the termination of the term for which the student is registered. Please see the UNT Schedule of Classes for details. You should return the evaluation to Dr. Emile Sahliyeh at sahliyeh@unt.edu. In the event that hours are not completed by the student by that time, you may submit your evaluation later and the student will receive an Incomplete in the course. After your evaluation, a grade change will be entered. Thank you again for allowing students to be a part of your organization.
